Purpose:
To provide linguistically competent and culturally sensitive primary health care services, particularly focusing on the underserved Asian American and Asian Pacific Islander population within Santa Clara County.

Description:
AACI's Primary Care Health Clinic is a fully-licensed community clinic serving all ages. The Clinic provides comprehensive primary and preventive health care and adopts a personal, attentive health care approach in its treatment of clients.

Services Offered:

  • Sick visits
  • Annual physical check-ups
  • Women's healthcare/annual exam
  • Breast health screening
  • Blood cholesterol screening
  • Diabetic screening
  • Children's healthcare (all ages)
  • Adult and youth immunization services, including flu shots
  • Health education and counseling

 

Eligibility:

  • Insurance plans accepted include Valley Health Plan, PPO, Selected HMO, MediCare, MediCal, and Healthy Families/Healthy Kids insurance.
  • Sliding scale fees are available for the uninsured and are determined by ability to pay based on income.
  • All ages are eligible.
